So when you do them with added weight, where should you hold the weight?

I know people say higher up on the body, and I know it makes it harder, but at the same time, this scares me even if he is using good leveredge since it's still so much weight.

Holding 20kgs on your chest is probably the equivalent of holding 5kgs above your head.
Weight is pretty irrelevant, but 210kgs sounds ridiculous.

He looks like he's holding the weights at his bellybutton, which I imagine would make it easier to lift that much weight.

I sometimes do decline sit ups with about 40 kilograms... You should definitely have someone there with you. If the weights slip and start crushing your head or something they can be helpful.

That's why I'd want to use a medicine ball or 2 handweights or something. How much of a decline?
